Adult acne affects approximately 25% of women and 12% of men over 25 — a population significantly underserved by aesthetic clinics that focus on cosmetic outcomes rather than the underlying pathology of acne. At Siam Dermatology, board-certified dermatologists from Thailand's Institute of Dermatology (the country's national derm referral center) treat acne as the medical condition it is: a chronic inflammatory disease of the pilosebaceous unit requiring targeted protocols, not generic facials.
What we treat
Comedonal acne (whiteheads, blackheads), inflammatory acne (papules, pustules), nodulocystic acne, adult hormonal acne (perimenstrual jawline flares, PCOS-associated), refractory acne (failed multiple OTC + GP regimens), post-inflammatory hyperpigmentation (especially in Fitzpatrick III-V skin), atrophic acne scars (icepick, rolling, boxcar), keloid scars on chest/back, fungal acne (Malassezia folliculitis), and acne in pregnancy.
Treatment ladder
Mild: Topical retinoid (Tretinoin 0.025-0.05% or Adapalene 0.1%) + Benzoyl peroxide 2.5-5% ± topical Clindamycin. Moderate: above + oral Doxycycline 100 mg/day x 12 weeks. Moderate-severe / nodulocystic / scarring: Oral Isotretinoin (Roaccutane) 0.5-1 mg/kg/day cumulative 120-150 mg/kg — with monthly LFT/lipid monitoring and pregnancy prevention (iPLEDGE-equivalent counseling). Adult female hormonal: Spironolactone 50-200 mg/day ± combined oral contraceptive (Yaz, Diane-35). In-office: Intralesional triamcinolone for cysts, chemical peels (salicylic, glycolic, TCA), fractional CO2/Er:YAG laser for scars, microneedling with PRP.
Why specialist vs aesthetic clinic
Aesthetic clinics commonly offer facials, light peels, and LED therapy. Specialist dermatology offers the full medical ladder — including Isotretinoin (requires board-certified dermatologist supervision in Thailand), Spironolactone, intralesional injection, fractional laser scar revision, and recognition of mimicking conditions (rosacea, fungal acne, perioral dermatitis, drug-induced acneiform eruption, hidradenitis suppurativa). Dr. Pat is Faculty at the Institute of Dermatology, board-certified (License #54323), and consults in English.
