Scleroderma (ผิวแข็ง) คือกลุ่มโรคที่ collagen สะสมมากผิดปกติ แบ่งเป็น 2 กลุ่ม — Localized scleroderma (Morphea) ผิวแข็งเฉพาะที่ผิว ไม่มี internal organ involvement — Systemic sclerosis (SSc) ผิวแข็ง + อวัยวะภายใน (ปอด ไต หัวใจ ทางเดินอาหาร)

Morphea (Localized Scleroderma)

Plaque morphea — รอยแผลรี ivory-white center + violaceous lilac ring active border Linear morphea (เด็ก) — รอยยาวตามอวัยวะ (en coup de sabre — หน้าผากลาง) Deep morphea — panniculitis Generalized morphea — ≥4 plaques in ≥2 anatomical sites Pansclerotic morphea — disabling Workup: ไม่มี Raynaud, ANA อาจบวก (boundary nuclear ssDNA antibodies), histones, anti-topoisomerase II

Systemic Sclerosis

Limited (CREST): Calcinosis cutis, Raynaud, Esophageal dysmotility, Sclerodactyly, Telangiectasias — anti-centromere positive Diffuse: skin + lungs (ILD), kidneys (renal crisis), heart — anti-Scl-70 (topoisomerase I) positive Anti-RNA polymerase III — เสี่ยง renal crisis + malignancy Nailfold capillaroscopy: dilated/giant capillaries + dropout = early SSc — ตรวจได้ที่คลินิกผิวหนัง

รักษา

Morphea: Topical/intralesional corticosteroid + tacrolimus 0.1% — limited plaques Methotrexate 0.3-0.6 mg/kg/wk + pulse prednisolone — generalized/linear/deep UVA1 phototherapy 50-100 J/cm2 — superficial morphea Hydroxychloroquine 200 mg bid — adjunct Physical therapy สำหรับ linear morphea ข้ามข้อ Systemic Sclerosis: ร่วมรักษากับรูมาติซั่ม—มี MTX, MMF, cyclophosphamide สำหรับ ILD; nifedipine + bosentan + iloprost สำหรับ Raynaud; ACEI สำหรับ renal crisis

Morphea จะกลายเป็น SSc ไหม? ไม่ — Morphea ไม่จะ progress ไปเป็น SSc แต่ linear morphea (เด็ก) อาจมี contracture + atrophy progressive ต้องรักษาเริ่มต้นเร็ว

Raynaud ต้องสงสัย SSc ทุกรายไหม? ไม่ — primary Raynaud มี 90% ไม่มี underlying แต่ถ้าบวกร่วมกับ digital ulcer + telangiectasia + nailfold capillary changes + ANA+ → SSc

UVA1 ได้ผลจริงหรือ? ได้ผลดีสำหรับ morphea ตื้น ช่วย collagen remodeling ประเทศไทยยังไม่มีแพร่หลาย — คลินิก IOD มี

MTX ต้องใช้นานแค่ไหน? Morphea generalized/linear/deep — induction 12-24 เดือน, taper สองปีถ้า inactive — sometime taper ไม่สำเร็จ→ long-term
