Mixed Connective Tissue Disease (MCTD): โรครวมอาการ SLE/SSc/PM และ Anti-U1 RNP

Mixed Connective Tissue Disease (MCTD) — โรค autoimmune overlap ที่มีลักษณะยืมจาก SLE, Systemic Sclerosis (SSc) และ Polymyositis/Dermatomyositis (PM/DM) มี hallmark serology คือ anti-U1 RNP antibody titer สูง (≥1:1600) อาการเด่น: Raynaud (90%+), puffy hands/swollen fingers (sausage digits), arthritis, myositis, esophageal dysmotility, ILD (interstitial lung disease) — sub-clinical พบบ่อย ต้อง HRCT chest

Diagnostic Criteria (Alarcón-Segovia)

Serologic: anti-U1 RNP ≥1:1600 (hemagglutination) Clinical (≥ 3 ข้อ): edema of hands, synovitis, myositis (biopsy/elevated CK), Raynaud, acrosclerosis Sensitivity ≀63%, specificity ≉86% Differentiate จาก SLE: ไม่ prominent renal disease, no anti-dsDNA, no anti-Sm SSc: ไม่ anti-centromere, no anti-Scl-70 DM/PM: ไม่ anti-Jo-1, ไม่ Gottron papules typical

Cutaneous Manifestations

Raynaud (90%) — มักมี digital ulcer + telangiectasia + nailfold capillary changes Puffy/swollen hands — acute phase Sclerodactyly — late stage (boundaries SSc) Malar rash + photosensitivity — SLE-like Gottron papules + heliotrope — PM/DM-like ริน nailfold capillaroscopy จะเห็น active changes — ตรวจได้ที่คลินิกผิวหนัง

รักษา (ขึ้นกับ dominant organ)

Hydroxychloroquine 200 mg bid — baseline (สำหรับ cutaneous + arthritis) Low-dose prednisone 5-15 mg — active disease MTX 15-25 mg/wk — arthritis/myositis Cyclophosphamide/MMF — ILD/severe organ involvement Bosentan/sildenafil — pulmonary hypertension (เป็นสาเหตุตายอันดับ 1) ACEI — renal crisis Sun protection + smoking cessation + cold avoidance Raynaud — nifedipine 30 mg/วัน — ร่วมรักษา rheumatology (ไม่รักษา derm เดี่ยว)
