Cutaneous Lupus Erythematosus (CLE) คือ autoimmune disease ที่ทำให้เกิดผื่นบนผิวหนัง แบ่งเป็น 3 ประเภทหลัก: 1) Acute Cutaneous LE (ACLE) — Malar rash รูปผีเสื้อบนหน้า มักร่วมกับ SLE 2) Subacute Cutaneous LE (SCLE) — ผื่นวงแหวนหรือ papulosquamous บริเวณ sun-exposed มักสัมพันธ์กับ anti-Ro/SS-A 3) Discoid LE (DLE) — แผลผื่นเป็นวง มี follicular plugging, atrophy, scarring alopecia

การวินิจฉัย — Workup ที่จำเป็น

Skin biopsy: Interface dermatitis with vacuolar change at DEJ, perivascular lymphocytic infiltrate, follicular plugging (DLE), basement membrane thickening, mucin deposition. Direct Immunofluorescence (DIF): Granular IgG/IgM/C3 at DEJ (Lupus band test — sensitive 70-90% in lesional skin). Serology: ANA (positive 60-80% in SCLE/DLE), anti-Ro/SS-A (75% SCLE), anti-La/SS-B, anti-dsDNA (specific for SLE), complement levels — เพื่อแยก systemic involvement

การรักษา

First-line topical: Sunscreen SPF 50+ broad-spectrum ทุกวัน (UVA + UVB) + Potent topical corticosteroid (Clobetasol 0.05%) หรือ Topical calcineurin inhibitor (Tacrolimus 0.1%) สำหรับใบหน้า. First-line systemic: Hydroxychloroquine (Plaquenil) 200-400 mg/day — ใช้เวลา 2-3 เดือนเห็นผล ต้องตรวจตา baseline + ทุก 5 ปี (retinal toxicity risk เพิ่มขึ้นที่ cumulative dose > 1000 g หรือใช้ > 5 ปี). Refractory: Methotrexate, Mycophenolate, Dapsone, Thalidomide (DLE)

เมื่อไรต้องส่งต่อ Rheumatologist

ผู้ป่วย CLE 5-25% จะมี progression ไป SLE ภายใน 5-10 ปี โดยเฉพาะ SCLE (>50%) — ส่งต่อเมื่อพบ: 1) Arthralgia/Arthritis 2) Constitutional symptoms (อ่อนเพลีย ไข้ น้ำหนักลด) 3) Proteinuria 4) Cytopenia 5) Anti-dsDNA positive 6) Low complement
