Azathioprine (ชื่อการค้า Imuran) — prodrug ของ 6-mercaptopurine (6-MP) → thioguanine nucleotides ยับยั้ง purine synthesis ลด lymphocyte proliferation ใช้ใน dermatology: severe atopic dermatitis, bullous pemphigoid, pemphigus vulgaris, lupus, dermatomyositis, pyoderma gangrenosum, vasculitis ˛ Dose: 1-3 mg/kg/day Onset: 6-12 สัปดาห์

TPMT / NUDT15 Testing — ต้องตรวจก่อนเสมอ

TPMT (Thiopurine Methyltransferase) ˛ ผู้ที่มี TPMT *3A/*3C variant (homozygous) → ไม่สามารถ metabolize ได้ → severe fatal myelosuppression (deep cytopenia + sepsis) ˛ 0.3% homozygous, 10% heterozygous (ลดขนาด 50%) ˛ NUDT15 R139C — สำคัญมากใน Asian (10-15%) มากกว่า Caucasian Pre-treatment: ตรวจ TPMT activity หรือ genotype พร้อม NUDT15 → ถ้า deficient → ห้ามใช้หรือลดขนาดมาก

Side Effects + Monitoring

GI: nausea, vomiting Hepatotoxicity: cholestatic hepatitis Pancreatitis (idiosyncratic) Cytopenia: leukopenia, anemia, thrombocytopenia Infection risk: HSV, VZV reactivation Malignancy: lymphoma 4x increased, NMSC 5x — ต้อง sunscreen + skin check ˛ Monitoring: CBC every 1-2 wk x 1 month, then monthly x 3, then every 3 months LFT, BUN/Cr same schedule

Drug Interactions — Allopurinol อันตราย

Allopurinol/Febuxostat ⊤ xanthine oxidase → เพิ่ม azathioprine level 4-5 เท่า → lethal cytopenia ˛ ถ้าต้องใช้ร่วม → ลดขนาด azathioprine 25-33% + close monitoring ACE inhibitor → anemia/leukopenia (ระวัง) Warfarin → anticoagulant effect ลด — ต้องตรวจ INR ถี่ Vaccine live (varicella, yellow fever, MMR) — contraindicated

Q: ถ้าตรวจ TPMT ไม่ได้ทำไง? A: ต้องเริ่ม dose ต่ำมาก (25-50 mg/day) + CBC รายสัปดาห์ช่วงแรก

Q: ขณะตั้งครรภ์ใช้ได้ไหม? A: Category D — ไม่แนะนำ แต่ถ้าจำเป็น (severe disease control) ใช้ได้ซึ่งปลอดภัยกว่า MMF/MTX (transplant guideline)
