Vitiligo Treatment with Ruxolitinib (Opzelura) in Thailand — FDA-Approved Repigmentation Therapy | Siam Dermatology

Vitiligo affects 0.5-1% of the global population. Until 2022, no FDA-approved targeted therapy existed for repigmentation. Ruxolitinib 1.5% cream (Opzelura) changed this — a topical JAK1/JAK2 inhibitor approved by the FDA for nonsegmental vitiligo in patients aged 12+. Treatment Protocol

Apply Ruxolitinib 1.5% cream twice daily to affected areas (limit 10% BSA). Treatment duration: 24 weeks minimum to assess response. Efficacy: F-VASI-75 (75% improvement in facial vitiligo) achieved in 30% of patients at 24 weeks; 50% at 52 weeks. Best responders: facial vitiligo, recent-onset (<5 years), patients aged 12-50.

Combination with NB-UVB Phototherapy

Combining topical Ruxolitinib with narrowband UVB (NB-UVB) phototherapy 3 times weekly enhances repigmentation rates significantly. Excimer Laser 308 nm is an alternative for localized patches. Topical Tacrolimus 0.1% or Pimecrolimus 1% provides maintenance after initial response.

Side Effects & Monitoring

Application site acne (6%), application site pruritus (5%), nasopharyngitis. Limit application to <10% BSA to minimize systemic absorption. JAK class boxed warning (thrombosis, MACE, malignancy) applies theoretically; topical dosing has minimal systemic exposure. Treatment is monitored by a board-certified dermatologist.
